IMPACT of the Piedmont (IMPACT) provides 24 hour a day, 365 days a year service to individuals who reside in our group homes in Davie and Yadkin Counties.

IMPACT can help you access the community! We can provide support for successful relationship building, volunteering, paid work, classwork or any aspect of discovering what gives your life purpose and meaning!

Support to meet the needs of individuals who require ongoing supervision, assistance to access the community, and/or personal care while primary care givers work or have a break.
Getting Started
We at IMPACT understand that when you need outside support, it can be overwhelming. Most people likely wonder, where do I start and question what do all of these terms even mean? We want to encourage you to breathe, know that support is available and take one step at a time. People will guide you through each step of the process.
You are not expected to know this path on your own. Soon, as you travel through the process, much will become familiar, and you will be navigating the arrangement of care like a pro! We have listed some suggestions of how to start your quest for services:

Request a screening for IDD Service Eligibility. To determine if eligibility requirements are met, you will be asked to provide documentation of the disability (which a doctor diagnosed before the age of 22) to include a psychological. The psychological evaluation must include IQ and adaptive functioning levels.

The next critical step in obtaining services is to have your name/or your family member's name added to the Registry of Unmet Needs. Partners, or your local MCO, will guide you through this process. The Registry is the gateway to funded services. While there is a wait for some services funding options, there are some services that may be available right away.
The most important message that we can give to you is to stress the importance of getting your name on the list so that your future is filled with options. You will not have access to funded services unless you get your name on the Registry of Unmet Needs.

With the application, please include a copy of the psychological evaluation, and Appointment of Guardianship, if applicable.
